As a new patient, please expect your 1st visit to last about 1 1/2 hours. I like to take some initial time to go over your health history and access your symptoms.
Then the actual treatment is an hour. If you would like to fill out your paperwork before coming in please print, and fill out the 3 forms located below.
Bring them in with you on your initial visit (along with your medical insurance card, if you are using your insurance), this will help to make your 1st visit run smoothly.
Please note that if you are using your medical insurance for payment, I encourage you to call your provider to check and see if a prescription is required by your Medical Doctor.
If an Rx is required, you will need to bring it in with you on your first visit so I can process your claim appropriately. Please don't hesitate to email me with any questions you may have.
Scheduling Scheduling is easy! Simply click on the "book now" icon located on any page and it will direct you to my online scheduling book. This will allow you to schedule appointments anytime of the day or week, 24/7. You will be able to find times that will work well with your personal schedule. Cancellation Policy Please allow 24 hours notice for all cancellations. Any missed or non-cancelled appointments will be billed in full to you, the client. Please note that your insurance company can not be billed for a missed appointment, it will be your responsibility.
